We are seeking a highly organized and customer-focused Bilingual Patient Coordinator who is fluent in English and Spanish to support the coordination of care for patients receiving home healthcare services.

The Patient Coordinator will serve as a key point of contact between patients, families, Home Health Aides (HHAs), healthcare professionals, contracts, and internal departments. The ideal candidate is an excellent communicator, comfortable working in a fast-paced healthcare environment, and experienced with HHA Exchange .

Schedule

Monday–Thursday: 9:00 AM–5:00 PM

Friday: 9:00 AM–4:00 PM

- 45-minute unpaid break each day

- Must be flexible to provide weekend on-call support as needed

Key Responsibilities

- Coordinate work assignments based on Home Care Worker training and experience.

- Communicate effectively with patients, family members, HHAs, clinicians, contracts, and other departments.

- Document all communications and relevant information accurately in HHA Exchange .

- Monitor and resolve open cases to ensure timely resolution.

- Support patients and families by identifying their needs and coordinating appropriate care.

- Communicate changes in service days/hours, discharges, and other relevant updates to HHAs.

- Provide necessary information to healthcare professionals and other stakeholders.

- Complete daily, weekly, and monthly reports.

- Manage overtime and travel-time requests.

- Use Smart Map and other tools to locate aides, provide directions, and check availability.

- Complete and document incident reports.

- Manage mutual and linked cases.

- Monitor and respond promptly to emails, voicemails, and faxes.

- Follow agency guidelines, policies, and procedures.

- Adapt quickly to changes within the agency and healthcare environment.

- Escalate concerns or questions to the appropriate supervisor.

- Perform other duties as assigned to support agency needs.

Requirements

- Fluent in English and Spanish — required.

- Previous HHA Exchange experience — required.

- Computer savvy and comfortable working with healthcare software and digital tools.

- Strong written and verbal communication skills.

- Excellent customer service and active listening skills.

- Strong organizational and time-management skills.

- Ability to multitask, prioritize, and manage multiple cases simultaneously.

- Ability to work effectively under pressure and in a fast-paced environment.

- Positive, team-oriented approach.

- Flexible and adaptable.

- Strong attention to detail and accurate documentation.
