Position Summary

The Pharmacist will be responsible for maintaining initiatives that ensure medications prescribed for kidney disease and its related comorbidities are evidence-based and cost-effective. The role involves addressing pharmacy referrals and assisting with medication access, education, and adherence to support kidney health management. The Pharmacist will work collaboratively with clinical and care management teams to identify medication opportunities and resolve medication safety concerns associated with transitions of care and chronic disease management.

Responsibilities

- Review medication regimens and make recommendations on how regimens might be adjusted based on protocols and current guidelines to optimize disease management

- Create written responses to pharmacy referrals related to medication access, affordability, education, adherence, optimization, and safety

- Evaluate medications that are high-cost, high-risk or high-volume using population health framework

- Assist with processes related to medication reconciliation and medication adherence

- Collaborate with other care providers in the effective sharing of practice expertise

- Oversee pharmacy technicians and pharmacists

- Participate in quality improvement activities and process enhancement initiatives as assigned

- Adhere to policies, procedures and state and federal regulations to ensure compliance and confidentiality of Protected Health Information (PHI). Participation in Compliance and other important training is a condition of employment

- Perform other duties as assigned

Requirements

- Active Pharmacist License

- PharmD degree from an accredited School of Pharmacy

- 5 years clinical pharmacy experience in working face to face with physicians and health care providers in hospital or ambulatory care

- Renal patient pharmacy management and value-based care experience, Health Plan and/or Vendor experience preferred

- American Society of Health Systems Pharmacists (ASHP) Accredited Residency preferred

- Understanding of Centers for Medicare and Medicaid Services (CMS) Star measures, Healthcare Effectiveness Data and Information Set (HEDIS) measures, MIPS and National Quality Forum measures

Skills

- Excellent written and verbal communication skills

- Strong organizational skills and attention to detail

- Ability to manage multiple tasks, solve problems, work independently

- Strong analytical and strategic skills

- Metrics driven

- Must be proficient in Microsoft Office: Outlook, Word, Excel, PowerPoint, Teams

- Bilingual Preferred- English/Spanish
