Nurse Practitioner, Home-Based Pulmonary Rehabilitation

Your Role...

As a Nurse Practitioner in our Home-Based Pulmonary Rehabilitation program, you will provide virtual clinical intake and assessment services for patients participating in Morlen Health ’s pulmonary rehabilitation program. You will evaluate patient history, clinical status, risk factors, and readiness for participation while using independent clinical judgment to identify concerns requiring additional review, coordination, or escalation.

In addition to direct patient-care responsibilities, you will contribute to Morlen Health ’s quality assurance, compliance, and medical informatics activities. You will support clinical quality initiatives, participate in the Quality Assurance Committee, and help implement medical informatics tools such as dashboards, patient surveys, and other measurement methods used to support clinical and program monitoring.

All remote positions are based in the United States, and candidates must reside within the U.S. to be eligible for consideration.

In This Role, You Will...

- Conduct comprehensive virtual intake assessments for patients entering Morlen Health ’s Home-Based Pulmonary Rehabilitation program.

- Review medical history, diagnoses, medications, clinical documentation, diagnostic information, and other relevant patient information to assess program readiness and clinical risk.

- Apply independent clinical judgment to identify contraindications, safety concerns, missing information, or issues requiring additional review, coordination, or escalation.

- Coordinate with referring providers, medical leadership, respiratory therapists, and other members of the interdisciplinary care team.

- Educate patients regarding program participation, expectations, safety considerations, and next steps while maintaining accurate and timely clinical documentation.

- Contribute to quality assurance activities across Morlen Health clinical programs, including chart reviews, documentation audits, case reviews, and quality monitoring.

- Identify clinical quality concerns, documentation gaps, or areas of risk and contribute to root-cause reviews, corrective actions, and follow-up activities.

- Serve as a member of Morlen Health ’s Quality Assurance Committee and provide clinical perspective to committee activities.

- Support clinical compliance activities, including policy and procedure review, clinical audits, regulatory readiness, and remediation of identified concerns.

- Support the hands-on implementation of medical informatics tools, including dashboards, patient surveys, and other measurement methods used for clinical and program monitoring.

- Partner with clinical and operational leaders to support adoption and effective use of informatics tools and emerging medical technologies.

- Foster a respectful and collaborative environment through professional, constructive, and solution-oriented communication.

What You Need to Be Successful...

- Master’s or doctoral degree from an accredited Nurse Practitioner program.

- Current, unrestricted Nurse Practitioner/APRN licensure in at least one state in which Morlen Health provides patient care. Licensure in the state of California is required.

- Ability to obtain and maintain additional state licensure based on Morlen Health business and patient-care requirements.

- Pulmonary, cardiopulmonary, respiratory, chronic disease management, rehabilitation, primary care, or related clinical experience.

- Current national Nurse Practitioner certification appropriate to the role and patient population and current BLS certification.

- Five or more years of progressive clinical experience as a Nurse Practitioner, including independent patient assessment and clinical decision-making.

- Experience with clinical quality assurance, quality improvement, healthcare compliance, clinical auditing, or a closely related function.
