Job Title: Home Team Nurse
Qualifications:

Education: Graduate of accredited school of nursing

Certification/Licensure: Valid, current Pennsylvania state license to practice professional/practical nursing

Experience: Previous experience in a clinic setting preferred

Alternative to minimum qualifications: None

General Summary of Duties: Provides professional nursing care under supervision for clinic patients in Transition of Care (TOC); triage communications between providers, patients, and office staff; address documents and update patient electronic health records and providers appropriately.

Supervision Received: Reports directly Nursing Administration
Supervision Exercised: None

Typical Physical Demands: Requires vision correctable to 20/20 and hearing to normal range. The ability to work at a computer for extended periods of time.

Typical Working Condition: Computer and telephonic communication with patients, providers, and office staff.

Examples of Duties – to include but not limited to:

Performs general nursing assessment of patients. Reinforces prescribed medication regimes and treatments in accordance with nursing standards.

Observes, records and reports patient’s condition and reaction to drugs and treatments to the physician.

Schedules appointments and ensures preferences are given to patients in emergency situations.

-

Screens patients for appropriate information. Instructs patients in collection of samples and tests as necessary.

Coordinates patient care with other providers, case managers, and ancillary services.

-

Responds to and directs incoming phone calls. Instructs patient and family regarding medication and treatment instruction. Instructs patients in self-care programs and participates in all aspects of patient education.

Updates, reviews and prepares patient charts.

Attends required meetings and participates in committees as requested.

Participates in professional development activities and maintains professional affiliations.
