How you make a difference
The Licensed Practical Nurse Telehealth assists in delivering patient care through assessment, planning, implementation, and evaluation under the supervision of a Registered Nurse. This role supports individualized treatment programs and guides patient teaching based on demonstrated competencies. The position assists onsite staff with remote access and virtual technology while providing care in accordance with established policies, procedures, and protocols. The Licensed Practical Nurse Telehealth also documents nursing encounters and communicates patient information to the appropriate healthcare team members. Key Responsibilities
- Assist with patient assessments, treatment planning, and patient teaching under the supervision of a Registered Nurse.
- Implement individualized treatment programs and therapeutic diet plans as directed by the Health Care Practitioner.
- Support onsite staff using remote access and virtual technology.
- Provide clinical and technical care in accordance with established policies, procedures, protocols, and safety practices.
- Document nursing encounters and communicate patient information to nursing staff, physicians, supervisory personnel, and other staff as necessary.
Qualifications & Requirements
Education
- Graduate from an accredited school of nursing.
Experience
- Prefer a minimum of one (1) year correctional healthcare experience or
- A minimum of one (1) year clinical experience.
Licenses/Certifications
- Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
- Must be able to obtain and maintain CPR certification.
