Nursing is the remote-healthcare lane with the most search demand and the least clarity. The postings exist — triage lines, telehealth practices, insurers and home-health operators all hire continuously — but almost every "remote nursing job" search lands on a single listing rather than a page that explains the market. This guide is that page: what can actually be done from home, which licence you need, what it pays, and who is hiring right now.

What can and cannot be remote in nursing

Remote nursing is consultative work. The clinical encounter happens over a phone, a video call or a screen of chart data, and the output is documentation. The lanes that show up in live listings:

  • Telehealth and telephone triage — patient calls, symptom assessment, advice lines and virtual visits.
  • Case management and utilisation review — insurers and health plans review claims and authorisations; this is the highest-volume corporate lane.
  • Care management and chronic-care follow-up — outreach, medication adherence, care plans for home-health and payer populations.
  • Remote patient monitoring — reviewing device and vitals dashboards, escalating deteriorating patients.
  • Nurse practitioner telehealth — independent assessment and prescribing, state by state.

What cannot be remote: bedside, theatre, ICU, and anything requiring hands-on assessment. If a posting is "remote" but the role is direct care, read the fine print — it is usually a hybrid or field role wearing the word as a tag.

The four title families, measured on our own index

Counting live postings whose titles carry the role (RemoShift index, measured 1 October 2026):

  • Nurse practitioner roles — about 404 postings titled NP, APRN, PMHNP or "nurse practitioner". The advanced-practice lane: highest pay, heaviest licensure.
  • Registered nurse roles — about 139 postings explicitly titled registered nurse, on top of the large pool of triage, case-management and care-manager postings that simply say "nurse".
  • LPN and LVN roles — about 63 postings. Real, but thin: the "remote LPN jobs" search cluster is one of the loudest in this space while the inventory is a fraction of the RN pool. If you are an LPN, expect fewer options and more utilisation-management-style duties than pure triage.
  • Everything else "nurse" — the remainder is mostly nurse navigators, nurse educators, RN care managers and clinical reviewers, the job titles that quietly hold most of the volume.

Licensure is the geography

Clinical work is regulated where the patient is, not where the nurse sits. A remote role serving Texas patients generally needs you licensed in Texas — so postings name states, and those lists are hard requirements, not preferences. Two things widen the net: the Nurse Licensure Compact, which lets one multistate licence cover participating states, and employers who hold multi-state licences or sponsor licence applications. Practically, that is why US-state-bound postings dominate the live list and why genuinely work-from-anywhere clinical nursing roles are a handful rather than a category. Filter by the states you already hold before anything else — it is the first screen every clinical recruiter applies.

What remote nursing jobs pay

Across the live postings that publish a salary (a sample of 245 disclosed figures, measured 1 October 2026): the 25th percentile sits near $71,100, the median near $78,800, and the 75th percentile near $116,400 a year. Live annual bands on the index run in the $65,000–$108,000 range for RN and care-management roles; nurse-practitioner and psychiatric telehealth postings sit above that band. A slice of the inventory is advertised hourly — triage, monitoring and per-diem coverage, typically $20–$60 an hour — and hourly clinical work is often part-time by design, so annualise it before comparing.

Who is hiring right now

The employer mix on the live index (1 October 2026) is three groups. National insurers and health plans — Humana, Centene, Oscar and peers — hire case-management and utilisation-review nurses in steady volume. Telehealth and digital-health providers — Lyra Health and a long tail of specialty practices — hire prescribers and triage nurses, often per-state. Staffing and home-health operators — firms like Lifelancer and regional home-health companies — post the highest number of individual openings. Expect three different rhythms: insurers hire continuously, digital health in bursts, agencies on client demand.

How to get hired

  1. Lead with licence, credential and states. "RN, compact, licensed in TX and OK" clears the first filter in one line; burying it costs you the screen.
  2. Name the specialty, not just the credential. "RN, utilisation review, oncology background" beats "registered nurse" in every keyword pass.
  3. Prove written communication. Remote clinical work is documentation. Clean, structured notes are the portfolio you can actually submit.
  4. Search the duty, not only the title. Most volume hides in "care manager", "utilisation review" and "clinical reviewer" postings that never say "nurse" in the query you typed.
  5. Apply state-filtered. Skip work-from-anywhere hopes in clinical lanes; filter for the states you hold licence in — that is where the live inventory actually is.
