Position Summary
The Vice President of House Calls is responsible for leading one of Mosaic's most critical value-based care programs, providing in-home clinical services to high-risk and medically complex patients. House Calls serves as a key wrap-around service that extends the primary care team into the home, improving patient outcomes, reducing avoidable utilization, and ensuring patients receive the right level of support at the right time.
This leader will drive exceptional patient care and operational performance through the management and growth of the House Calls program, with a particular focus on patients with advanced chronic illness, functional decline, and those requiring increased care coordination. The VP will work closely with primary care providers, care management, palliative care, hospice partners, and interdisciplinary teams to deliver comprehensive, patient-centered care.
The VP of House Calls is responsible for continuous improvement in operational fundamentals including provider productivity, schedule optimization, workforce planning, recruiting and developing clinical teams, quality outcomes, patient engagement, and financial performance. The role is accountable for driving growth through strong provider engagement, referral optimization, and strategic alignment with regional operations.
As a key leader within Mosaic's value-based care model, the VP will play a critical role in advancing the organization's End-of-Life (EOL) strategy by ensuring high-risk patients are appropriately identified, engaged, and connected to the most appropriate wrap-around services, including House Calls, palliative care, hospice, and other supportive care programs. This leader will help create clinical pathways that improve quality of life, align care with patient goals, and reduce unnecessary healthcare utilization.
The VP will hold full accountability for the budgeting, operations, growth, quality outcomes, and EBITDA performance of the House Calls program, ensuring high-quality, cost-effective care within a full-risk healthcare environment.
P&L and Operational Management
- Maintain full operational and budgetary responsibility for the business unit, driving strategies to meet or exceed EBITDA targets.
- Develop and execute growth strategies that expand access to House Calls services for high-risk and medically complex patients.
- Monitor and improve operational performance through effective workforce planning, provider productivity, schedule optimization, and resource allocation.
Clinical Leadership & Strategy
- Develop and implement clinical initiatives and evidence-based pathways across Mosaic to enhance patient outcomes while reducing total cost of care.
- Lead the organization's strategy for delivering in-home care to high-risk, medically complex, and advanced illness populations.
- Partner with primary care providers, care management, palliative care, hospice partners, and regional leadership to ensure patients receive the appropriate wrap-around services as their needs evolve.
- Advance Mosaic's End-of-Life (EOL) strategy by establishing processes that identify and engage patients who would benefit from House Calls, palliative care, hospice, and other supportive care programs.
