GENERAL STATEMENT OF JOB
The Transitions to Community Living (TCL) Community Liaison’s primary objectives are to provide education to members/recipients and entities stakeholders about TCL, educating providers on how to identify and report barriers, aid in eliminating barriers, provide and create training opportunities and how to access TCL and via the RSVP (Referral Screening Verification Program) process. This position works with various community stakeholders such as the individual, family, LRPs, community/state hospitals, providers, shelters, and adult care homes. This position will work closely and collaboratively with members of the TCL Team in order to meet the established time frames with requests for required documentation as needed through the process. This position ensures that the specific functions related to the Department of Justice and RSVP are followed. This is a mobile position that will require working in various locations throughout and beyond Vaya Health ’s catchment area.
ESSENTIAL JOB FUNCTIONS
Community Education/Liaison Responsibilities:
A high level of diplomacy and discretion is required to effectively create buy in, negotiate, and resolve concerns or issues, this requires exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts. Problem solving, and conflict resolution skills are essential. Demonstrated effective leadership presence is required, with demonstrated confidence in dealing with unknown and complex issues. Awareness of and sensitivity to the complexities of stakeholder and participant concerns.
- Educate individuals members/recipients and referral entities (hospitals, providers, FCH, ACH, ALF and other key stakeholders) about the TCL eligibility requirements and the RSVP process.
- Educate and provide training opportunities to internal and external stakeholders regarding TCL program process and how to identify and report barriers.
- Cascade information to applicable departments within Vaya Health (Vaya) to assist with eliminating TCL barriers
- Coordinates with referral entities to address and assess the needs of the members/recipients, educate on appropriate services and levels of care as needed.
- Identifies gaps in services and intervenes to ensure that the individual receives appropriate care, can refer to MHSU Care Management, if appropriate.
- Ensures that services for the individual are identified and coordinated across the LME/MCO’s system and with other systems, including primary care.
- Provides education and assistance in collaboration with local hospitals (and other stakeholders) as appropriate to RSVP.
- While providing outreach and education in the community, the Liaison may also provide potential members/recipients with information on TCL process.
- Educate facilities stakeholders when members/recipients do not meet TCL eligibility and provide additional community resources when inquiries are received.
- Other specific functions as they relate to Diversion, In-Reach, Transition Process and Post-Transition responsibilities as indicated from DMA/DMH.
- Performs related tasks as required.
Collaboration :
- Collaborates with other members of TCL team and the community to identify areas of need and provides outreach and education in these areas.
- Serves as a collaborative partner in identifying system barriers. Attends monthly Vaya Local Barriers Committee as a “voting” member to discuss, collaborate and resolve TCL barriers.
- Sets up, manages, and facilitates meetings with community stakeholders as appropriate to TCL and RSVP process.
- Facilitates and coordinates the quarterly ACT/CST Collaborative Meeting with assistance acros
