Principal Care Management (PCM) Nurse
This is a remote position.
Only LATAM-based candidates (bilingual in Spanish and English)
Location: Latin America (Remote)
Schedule: Full-time โ U.S. business hours (EST)
Payment: $1,200 USD/month (depending on experience)
We are seeking a licensed nurse or MD to support a U.S.-based endocrinology practice through its Principal Care Management (PCM) program .
PCM is a care coordination program designed for patients with a single chronic condition requiring ongoing management . In this role, you will work closely with the provider to ensure patients receive continuous support between visits, helping improve outcomes and adherence to treatment plans.
This is a clinical, patient-facing role , requiring strong judgment, communication skills, and the ability to actively monitor and guide patients throughout their care journey.
Key Responsibilities
- Maintain ongoing communication with patients enrolled in the PCM program
- Monitor patient conditions, including:
- Symptoms
- Medication adherence
- Progress and response to treatment
- Provide patient education regarding their condition and care plan
- Assist in the implementation and follow-up of care plans
- Coordinate services and ensure continuity of care
- Identify changes in patient status and escalate concerns to the provider when needed
- Ensure patients follow treatment recommendations and scheduled appointments
- Accurately document all patient interactions and updates in the system
Requirements
- Licensed Nurse or MD (required)

Advanced English level (C1 or higher) โ excellent spoken and written communication
- Proven experience working with U.S.-based medical offices
- Experience with chronic care management, patient follow-up, or care coordination
- Strong clinical judgment and ability to assess patient conditions remotely
- Experience providing patient education and support
- Highly organized with strong documentation skills
- Comfortable working independently in a remote environment

Schedule: Monday through Friday, 9:00 a.m. till 7:00 p.m. Florida time (ES).

Work Hours: 45 hours per week.

Breaks: 1-hour lunch break and one 30-minute break per shift.
