Role Overview

Join Remedy Home Health Care as a licensed Physical Therapist to provide high-quality rehab and community-based care to patients in their homes. Work with senior care populations, including geriatric patients recovering from illness, injury, or surgery.
What You Will Do

Conduct in-home physical therapy evaluations and reassessments, develop and implement personalized rehabilitation care plans, and provide therapy in home health, assisted living, and community care settings.

Requirements

- Licensed Physical Therapist (PT) – California

- CPR Certification (AHA approved)

- Minimum 1 year of clinical experience (Home Health preferred)

- Experience in senior care, geriatrics, rehab, or community-based care preferred

- Strong clinical, communication, and patient education skills

- Valid driver’s license, reliable transportation, and auto insurance
