This fully remote position plays a critical role in helping individuals access and navigate behavioral health, substance use, medical, and community-based services that support long-term recovery and wellness.

Job Summary
The Case Manager is responsible for coordinating and managing client care services through the Center of Excellence program. This role involves working closely with clients, providers, community agencies, and interdisciplinary team members to ensure clients receive comprehensive, person-centered support. The Case Manager assists with care planning, resource linkage, treatment engagement, and ongoing follow-up to promote positive outcomes and continuity of care.
Essential Duties & Responsibilities
- Coordinate client care and service delivery through ongoing communication with healthcare providers, behavioral health professionals, community agencies, and support systems
- Conduct outreach and maintain regular contact with clients to support engagement in treatment and recovery services
- Assess client needs and assist with referrals to medical, behavioral health, housing, employment, transportation, social service, and recovery support resources
- Develop, implement, and monitor individualized care coordination plans
- Support clients in overcoming barriers to treatment, recovery, and overall wellness
- Collaborate with multidisciplinary treatment teams to ensure continuity and quality of care
- Assist with appointment scheduling, follow-up activities, and care transitions
- Maintain communication with referral sources and community partners as appropriate
- Document all client interactions, care coordination activities, and service plans in accordance with program standards and regulatory requirements
- Monitor client progress and identify opportunities for additional support services
- Participate in team meetings, case reviews, training programs, and professional development opportunities
- Maintain knowledge of available community resources and healthcare services throughout Pennsylvania
- Perform additional duties as assigned
Qualifications
Education & Experience
- High School Diploma or GED required
- Associate’s or Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, Healthcare, or a related field preferred
- Minimum of one year of experience in behavioral health, substance use treatment, mental health, healthcare, case management, or related services preferred
- Experience working with individuals experiencing substance use disorders preferred
- Valid driver's license preferred
